Our Process
Our Full Service Evaluation Process At Bluebird Wellness KC
First step is obtaining your medical records or a summary of your condition for our doctor to review. If you have them on hand you can email, fax, or bring them into our office or we can request them from your doctor for you.
Book Online
Next, we will need you to fill out a short intake form.
ID
We also need the following per state requirements:
- –Government/State ID
- -MO ID card, vehicle registration, utility bill, phone bill, internet bill
- -Proof of residency valid MO driver’s license,
- -Clear, color photo of the qualifying patient’s face.
Certify
Last step is to upload your documents and certificate and pay your fees. The state fee for a patient license is $25.58, caregiver license is $25.58, and patient cultivator license is $102.30 plus applicable taxes and card fees. Our new patient fees (includes filing) $150, Renewal fees (includes filing) $125. Physician’s certification is $100 without filing. **Veterans/Disabled 10% discount.
