  • Have a diagnosis of a qualifying medical condition – refer to the list of approved conditions.
  • Obtain an official certification from a doctor.
  • Be a resident of Arkansas and provide proof of residency, such as an Arkansas driver’s license or state ID.

Note that members of the Arkansas National Guard and the U.S. Military are prohibited by state law from obtaining a registry ID card.
